Treatment for Odontoid Fractures Using Anterior Screw Fixation



Abstract
fractures of the odontoid process were previously treated by suing non-operative or operative methods with varying degrees of success. There are many controversies in the treatment of type Ii fracture by Anderson and D' Alonzo classification due
to
high
incidence of nonunion. In type Ii ondontoid fractures. Most of studies recommended primary posterior C1-2 arthrodesis as the initial treatment. These posterior C1-2 fusion compromise axial rotation, flexion and extention.
Anterior screw fixations were performed in 10 cases of type Ii odontoid fractures to solve these problems. Bony unions were achieved within 10 to 16 weeks(average 12 weeks) after surgery without limitation of motion. Complications developed in 4
cases,
three were transient dysphasia and one loosening of a screw and displacement of fracture fragment. However bony union was achieved 16 weeks after surgery by using more rigid orthosis.
We conclude an anterior screw fixation could be used as a successful treatment for type II odontoid fractures.
